Medicare Agencies and Assistance Programs

Medicare questions are handled by several federal, state, and private organizations. Knowing which organization owns the issue can prevent repeated calls and reduce the chance of relying on an unofficial answer.

CMS and Medicare

The Centers for Medicare & Medicaid Services is the federal agency that administers Medicare. Medicare.gov and 1-800-MEDICARE provide official coverage, plan, claim, rights, and general program information.

CMS policy may be implemented through Medicare contractors and private plans.

Verify: Use official Medicare channels for general program rules, claims questions, and plan comparison.

Social Security

The Social Security Administration handles many applications for Part A and Part B, premium billing matters, income-related adjustments, late-penalty determinations, and applications for Extra Help with drug costs.

Social Security does not operate provider networks or decide every plan claim.

Verify: Direct enrollment and premium-record questions to Social Security.

Medicaid

Medicaid is a joint federal-state program administered by states under federal requirements. It may help eligible people with Medicare premiums, cost sharing, and additional services.

Eligibility categories, benefits, provider participation, and renewal processes vary by state.

Verify: Contact the state Medicaid agency for the individual case.

SHIP

State Health Insurance Assistance Programs provide free, unbiased counseling to people with Medicare and their families. SHIP counselors can explain options, help compare coverage, and assist with certain appeals or problems.

SHIP is not an insurance company and does not sell plans.

Verify: Use the national SHIP locator to find the local program.

Extra Help

Extra Help is a federal program that helps eligible people with Medicare prescription drug costs. Social Security generally handles applications, while some people qualify automatically through other assistance status.

Extra Help is not the same as Medicaid, although eligibility can overlap.

Verify: Use Social Security’s current Extra Help information and keep the determination notice.

Plans and insurers

Medicare Advantage, Part D, and Medigap insurers answer questions about their own contracts, applications, premiums, networks, formularies, claims, and authorization rules.

A plan representative can explain the plan but cannot redefine Medicare law or another company’s contract.

Verify: Request the answer in writing and identify the controlling plan document.

Providers and pharmacies

Providers explain clinical recommendations, scheduling, billing codes, Medicare participation, and whether they accept a particular plan. Pharmacies can explain dispensing and claim results.

They may not know every future plan rule or enrollment consequence.

Verify: Verify coverage and authorization with the plan as well as the provider.

Licensed agents

A licensed agent can provide individualized insurance assistance within licensing, appointment, and compliance requirements. An agent is not Medicare or a government official.

Educational content should remain separate from a voluntary request for personal assistance.

Verify: Use a separate consent-based assistance pathway and verify credentials.

State insurance departments and legal help

State insurance departments regulate insurance companies and can provide complaint or licensing information. Legal questions about powers of attorney, estates, or rights may require a qualified attorney.

SHIP counseling and legal advice are not the same service.

Verify: Choose the organization based on the actual question and jurisdiction.

Common misunderstandings

  • “Medicare and Social Security are the same agency.” They have different roles.
  • “SHIP sells Medicare plans.” SHIP provides free counseling.
  • “An insurance agent represents the government.” A licensed agent is not a government official.

  • Is this an enrollment, premium, coverage, claim, plan, Medicaid, or legal question?
  • Which organization issued the notice?
  • Does the question concern one specific plan contract?
  • Would free SHIP counseling help?
  • Do I need an official determination in writing?
